We have selected Guardian Life as our dental insurance carrier for this year’s dental renewal. Premiums for this plan are guaranteed from April 1st, 2010 to March 31, 2011. If you are a current dental plan member, you will automatically be enrolled into this new plan.

- Benefits for ANY dentist - out of network paid to 90th percentile!! Maximum benefit for out of network limited to $1000 per calendar year. A combination of in-network & out-of-network dentists may be used for different procedures.
- Vision discounts for members through the VSP network.
- New! Automatic payment option: Monthly electronic withdrawals (ACH) from your checking/saving account.
Manual monthly administrative fee is $10 per statement. Manual quarterly administrative fee is $20 per statement. To reduce the administrative cost associated with bringing you this plan, please complete the ACH form, include a voided check or savings deposit form; send it back to us by email, fax or via mail.
